Comply foam eartips are a great upgrade over the stock silicon eartips included with most earphones / IEMs. Where I find that silicon eartips become uncomfortable within 60 minutes (if I'm lucky) or 10 minutes (if I'm not lucky or the eartip fit / design is poor), I've not yet had any comfort issues with the Comply T-500 tips.
One thing to note is that these tips have a tendency to slightly muffle high treble coming out of the earphones. This can be a negative or positive depending on how "bright" the earphone's sound is. The Comply tips also make it much easier to get a good seal in the ear, which is essential for most earphones to get realistic bass response.
(Note: I use black T500s with Ultimate Ears 700 earphones. In this case I found the slight reduction in treble to be a plus.)

I won't say that these are completely flawless, but they're the best I've seen so far.
Once in a while one of the companies will furnish earbud pieces that actually stay in, and
seal well. (Strangely, my last pair of buds with excellent ear-piece fittings were a
cheap Sony version I picked up in the city on discount --I have never since encountered a
better fit, even in Sennheiser, Koss, you name it.) These foam tips make up for it all.
They work great. My only issue is that if you exercise they occasionally need to be
pulled out and reseated. Not sure if it is body temperature change or something else.
Adjusting will not do. You need to actually (and very simply) pull them out and reseat
them, and the fit returns immediately.
The pack comes with three sets of tips. I am still working on my first set. Do not be
alarmed when the tips seem to become misshapen, which is normal. I believe someone else
here mentioned a way to clean them. I have not had to do that yet. I figure that if the
three sets of tips give me a full year or more of use (changing them every four months or
so), then they are well worth the bucks.

These fit perfectly on my Altec Lansing UHP336 earphones, and the sound is wonderful. They are not totally sound isolating, but for a recent flight across the country they enabled me to listen clearly to my iPod while doing a good job of blocking out the baby that was three rows away, and the steady hum of the engines. I totally recommend them - they are used exactly like shooter plugs.
